Below is a selection of carrier websites where you will find the forms required to make changes or additions to your plan. To change your current benefit plan, please contact us directly so we can help. Once you have completed an administrative form please email or fax it to our office so that we can make sure it is completed correctly and executed in a timely manner.

Renewals

We have just had our renewal? Do we have to wait until our next renewal to look at other carriers or insurance companies?

Absolutely not. Employee benefit plans run from month to month. You are obligated to provide your current carrier with a minimum of 31 days written notice if you wish to terminate.
